Q: HI ,I am 64 year age, does the level 10 of PSA indicates a malignant Prostate growth for sure ? Thanks.?
I have no urine problems ,no urine retention,no dribbles, what actions should I take to be on the safe side .Thanks

A: Your doctor should have explained the results to you. A PSA is not a diagnostic test so nothing about the results are for sure. You should discuss this with your urologist. If you want to be safe you need the prostate removed.

Q: The PSA level of my father decrease from 10.9 to 4.8 .Is it a good symptoms?

A: I think I have answered your other question somewhere. I know you are concern about your father’s PSA. PSA can go down if you take some medication like Proscar. PSA is not a symptom and if it comes down with medication, it tells you nothing.

In the total picture it is important to know how old is your father? did he has history of prostate cancer in the family? Had he had previous diagnosis of prostate cancer?

There are few signs of prostate cancer: On rectal exam, you can sometimes differentiate benign or malignant prostate; Trans-rectal ultrasound can give you pathognomonic sign of cancer….But the final diagnosis is by tissue diagnosis by trans rectal needle biopsy under ultrasound guidance.

I hope your father is being followed by a Urologist. The PSA is only a test and it will not tell you that a prostate cancer is cured or improving unless after a radical prostatectomy. Also PSA between 4 – 10 do not necessarily tell you that the person has prostate cancer either.

Q: What are the odds of surviving Gleason 9 prostate cancer at age 56?
A friend of mine, who is 56 years old, was just diagnosed with prostate cancer. He has a pretty advanced case, gleason grade 9 and I think a PSA under 10. His dad died of prostate cancer. The doctors are worried because a year ago, he was tested and had extremely low PSA levels, but now they are much higher. This could be a sign of aggression. They don’t know if it has spread yet. His doctor has recommended surgery in Seattle.
Has anyone gone through this? Can anyone tell me what the likelihood is that he will be OK?
Oh, just found out the PSA last year was 2 and now is 5.

A: The Gleason score “grades” tumors on the basis of how abnormal they look when the tissue is examined under a microscope. The more abnormal they look, the more likely they are to grow fast and to spread to other parts of the body. A grade of less than 4 generally means that cancer cells look similar to normal cells. Grades 5 to 7 fall under the intermediate range. Grades 8 to 10 indicate aggressive growth. survival rate is an average of 5 years.i would have a second opinion, and maybe some more tests such as a cat scan, bone scan(this shows what is called (hot spots) to show advancement and a biopsy to examine types.the grading can get complicated what you do need to ask is what his T-score is or TNM this tells basically if a primary Tumor Localized) n is for spread into lymph Nodes (regional)and M Metastatic into other bones(stage 4) or organs.then life expectancy is calculated for each stage. let me know what his Tscore is and email me with any other facts or test results. the best thing you can do is support your friend on his choices and through his recovery.

Q: What ever happened to the really significant PSA’s? Why were they taken off the air?
Special thanks goes out to Nick and Don’t Panic for their contributions. As soon as Y/A allows me to choose a best answer you are both in like flynn and get your much deserved 10 points for best answer.

For those of you too young to remember with PSA’s like this… (thank you Nick)

Why in the heck are PSA’s like this no longer aired? I am 37 years old. I have seen a plethora of PSA’s about drinking and driving and smoking. And even pot. Though I find it interesting that even those commercials are becoming fewer and more far between.

Why is it that something as important as the air that we breathe, gard drugs like heroin, and even milder drugs like pot, are becoming or HAVE become a taboo subject?

People die in masses every day from heroin overdoses. Countless trees are cut down to build another parking lot. Pot has practically become as easy to buy as cigarettes.

Why in the sam hell are these PSA’s no longer aired? Am I the only one that sees a huge red flag here? When the comfort level of the people’s reaction to a subject matter becomes more important than the people dying from the subject in question instead?

A: *Sigh*. I miss them too. Now EVERY PSA is about “don’t smoke!” I mean LAME! I would rather my kids smoke than do drugs. The world just doesn’t care anymore. I am so sick of no smoking commercials aimed at adults. What about the kids? Isn’t that what PSA announcements were created for? Eh, the world sucks now.

Q: Unicornians: Do you accept these self-evident truths?
unicorns are scriptural – but they were like a massive animal like the rhino with its single horn

Num 23:22 God brought them out of Egypt; he hath as it were the strength of an unicorn.
Num 24:8 God brought him forth out of Egypt; he hath as it were the strength of an unicorn: he shall eat up the nations his enemies, and shall break their bones, and pierce [them] through with his arrows.
Deu 33:17 His glory [is like] the firstling of his bullock, and his horns [are like] the horns of unicorns: with them he shall push the people together to the ends of the earth: and they [are] the ten thousands of Ephraim, and they [are] the thousands of Manasseh.
Job 39:9 Will the unicorn be willing to serve thee, or abide by thy crib?
Job 39:10 Canst thou bind the unicorn with his band in the furrow? or will he harrow the valleys after thee?
Psa 22:21 Save me from the lion’s mouth: for thou hast heard me from the horns of the unicorns.
Psa 29:6 He maketh them also to skip like a calf; Lebanon and Sirion like a young unicorn.
Psa 92:10 But my horn shalt thou exalt like [the horn of] an unicorn: I shall be anointed with fresh oil.
Isa 34:7 And the unicorns shall come down with them, and the bullocks with the bulls; and their land shall be soaked with blood, and their dust made fat with fatness.

It doesn’t read exactly the same as my own version, but I was hoping there was actually some level of correlation… And I apologize that my level of ‘divine discernment’ isn’t that of a true Unicornian…

A: heresy!

Q: English native? help me~ for my letter~?
I’m writing a motivation letter for a master program. Dose my motivation letter sound reasonable and logical? How to improve it?

My name is ABC, and I attained a pass for my studies from the Mechanical Design, Manufacture and Automation Faculty in the in 123 University (top 10 in China), which houses the world first-class level ARCELOR MITTAL Metal Research Institute. I worked with XXX as an R&D Engineer since 2008.

A thorough understanding of the training system of Grandes Ecoles d’ingénieur (GE) made me decide on pursuing a master degree in GE. To begin with, the training orientation of GE is in line with my future career plan. As an engineer who intends to engage in R&D or project management in the industry after a study of two years, what any student expects most is the advanced know-how, technology, and practical skills, rather than theoretical researches. The curriculum of GE is practical and profession-orientated, and integrates the training for the social and professional world, which distinguishes French GE from programs offered by French public universities and elsewhere. The short internships and projects between each semester and the long internship in semester four introduce graduates to professional life effectively, and provide me with opportunities of knowing the operation of French industry, broadening my vision and knowledge, and being an internationalized industrial professional.

Moreover, France is the country I desire the most. The brilliant French achievements in art have been highly appreciated by me since I was 5 years old when I started learning to play the accordion. Especially, in the field of my major—mechanical engineering, France has a good reputation for its high technology and plays a key role in worldwide. Included are the leading manufacturers in France – Airbus, PSA, Renault, Saint-Gobain, as well as the company I am working for: XXX. Such a perfect country with well synthesized arts and engineering is not only significantly attractive, but also exclusive in the world. Through trainings offered by XXX, I acquired the French language and French engineering rules, as well as good cooperation from French engineers, and I believe learning and getting along with French would be an enjoyable experience.

With the rapid development of Chinese manufacturing industry, increasing numbers of French enterprises locate new branches in China, and therefore, there is a huge shortage of engineering talents who have experiences in both China and France. With a qualification attained at GE, I believe I could fill this role ideally in the future.

A: It looked pretty good overall, but I just had a few aesthetic changes and comments. I put the changes in [] and the comments in {}.

Does enlargement of prostat has any relation with erectile dysfunction?

A: Your Investigation are more or less normal (ECG I can’t inerpret without its strip). BMI 25 means you are not abese. Your sugar is high but decreasing HbA1C means you brining it to normaly. PSA is normal, Lipid profile is suggestive of sedentry lifestyle. why you are taking tamsulocin? What all urinary symptoms you have? 34 grms is not a very big size.
Your erectile dysfunction could be due to Diabetes (if you have a long history of DM), or due to prostate anlargement, or just psychogenic, it is just not possibe to make diagnosis only by investigations, without proper history and examination (tamsolcin usually is not a caus eof ED). Yeah one more thing: dont go for prostate surgery for erectile dysfunction, it only improves urinary symptoms.
Testosterone injections are only prescribed when its value is low (you require Serum testosteone/ FSH and LH value)
Best wishes

Q: Would you please help to improve my English letter?
I am writing a motivation letter in English for applying a master program in France. Would you please help me to improve it, and make it being professional, high-level, not redundant, and more like a paper written by an master degree holder? Thank you~

My name is xx. I graduated from the Mechanical Design, Manufacture and Automation in xx University, which is in the top 10 of China. Thanks to my outstanding performance in the university, I joined in ABC as an R&D Engineer. In the three years of work experience, I developed a clear direction of my future career and I am aware of the importance of shaping a widely international view, and therefore, I make a decision of pursuing my study in France. The reason why I expect to study in France is as below.

To begin with, France is the country I have desired mostly. I knew France by learning accordion since 5 years old. The brilliant achievements of France in art are highly appreciated by me since I was a child. Meanwhile, in the filed of my major—mechanical engineering, France plays a key role all over the world. France is world’s fifth largest economy and has a well reputation for high technology in mechanical filed. The design conception, the production management and the research and development of French manufacturing are all on the leading positions of the world. There are the most famous aircraft manufacturer—Airbus, and the top advanced automotive brand—PSA Renault, as well as developed auto parts industry—such as the company I am working in— ABC. Such a perfect country which well synthesized arts and engineering is not only significantly attractive but exclusive in the world.

In addition, when I joined in ABC, I recognized not only the strong of French engineering, but also the high quality of education in France, especially the Grandes Ecoles d’ingénieur. My French colleagues who graduated from Grandes Ecoles present solid foundation of mathematic, practical ability, combined with logical thinking ability, which indicates the high standard training offered by Grandes Ecoles d’ingénieur evidently.

In the light of these reasons, pursuing a master degree in France would be my only choice and desire. Moreover, I believe I have the ability to go through this program. Firstly, I am familiar with French mechanical regulars. I jointed the French company – ABC –as soon as I graduated from the university, and therefore, my entire work method is following the French model. Through the on job training offered the supervisor in ABC, I learned the French design rules, mechanical standard ( such as NTF), project management, quality controls, CAD standard, etc. In the meantime, I know the way to communicate and work with French engineers as well. During these three-year working, not only I acquire French, but also I made friends with French. Their enthusiasm, hard-working and romantic personalities made good impressions and I believe learning and getting along with French is an enjoyable thing.

Compared with the lacking of direction when I was fresh out of the university, there-year-working provides me the ability to make a mature decision of pursuing studies in France. I believe my study in France would be beneficial, well-judged and exciting. I am confident that I would be excellent in this program and France Grandes Ecoles d’ingénieur can offered me valuable training which can be of great benefit to my future career.

A: Well, you can always do like this:

I graduated from the Mechanical Design, Manufacture and Automation in xx University, which is in the top 10 of China. Thanks to my outstanding performance in the university, I joined in ABC as an R&D Engineer.

rendering to this:

I completed my study from the Mechanical Design, Manufacture and Automation in xx University, Which is in the pinnacle 10 of China. Much obliged to my marvellous rendition in the university. I teamed up ABC as an R&D Engineer.

Q: Is our congress govt capable of handeling big ticket project?
During NDA time there were two BIG ticket projects which were handled with perfection .One is Golden Quadral and second is Prime minister rural road project.

In last UPA govt the first thing it does was to virtualy halt the progress of these two projects under T.R.Balu…Secondly it anounces three big icket project one is Bharat Nirman ,and second is NREGA and third one is JNURM (Jawahar Lal Nehru urban renewal mission)

Lets talk about Nrega first, it looks great on papers and cosy to discuss in drawing room but leaving no impact on general below poverty line person.Actually i would call it a waste of Tax payers money. We do not have any mechanism and accountability factor how this money is spent and how much it actually goes in the hand of the needful.

http://www.indiatogether.org/2007/may/psa-nregs.htm

Bharat Nirman :–i don,t want to say anything anybody can read it in the below link

http://www.indianexpress.com/news/how-bharat-nirman-has-crumbled/495909/

Jawahar Lal Nehru urban renewal mission:—It was meant for the renewal of the infrastructure level of Metros and cities, but any body who visits the metro of India can see where the average tax payers money is going. You have slums ,Cities with a power cut of 5-10 hours per day. In the capital it is 2-3 hours per day…

Now the question where the money is going that is allocated .Is congress is making fool of AAM ADMI again, because it,s in the ineterst of congress that People of India should remain Poor so that they can be make fools again and again, for congress advantage..

Levae these projects they are not being able to execute a small game project such as commonwealth games precisely,,how could it handle any big projects…

A: They are handling one project with efficiency for almost 60 years. GNFPIP . Gandhi-Nehru Fool the People of India Project.
